All required SFUSD pre-participation paperwork must be submitted to the coaches on Tuesday November 1 at 3:45 pm on Feibusch Field: Physicals (Exam & History), Parent Consent, Anti-Steroid, Concussion Information, and Cardiac Arrest Information (available on this site under /Forms).

Club players not eligible to tryout until November 28 (3:45 pm on Feibusch Field) must also submit their completed paperwork o November 1 at 3:45 pm at Feibusch Field.
